1/12/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Laylah upon arrival to the home setting. Upon arrival to the home of the setting, Laylah appeared to be in a content mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. She verbalized to QMHP-C that her day was going well. QMHP-C informed Laylah that this session was going to focus on anger management and communication skills at which mother reports behaviors last from 2-4 minutes in duration on a daily basis when she cannot have her way. 2. Behavior: Laylah appeared to be irritated when she seen her mother’s boyfriend come in the door. Laylah indicated that she did not want to interact with her mother’s boyfriend because he is annoying and he is controlling. Mother reports behaviors last at least 2-4 minutes in moderation when she cannot have her needs met. 1.Intervention: QMHP-C began session by inquiring about Laylah’s emotional wellbeing. QMHP-C discussed positive character traits versus negative character traits, using Laylah’s responses from a previous interventions (ABC model) to categorize traits as negative/positive. QMHP-C and Laylah role played and discussed character scenario cards such as how to demonstrate cooperation, trust and empathy. 2. Intervention: QMHP-C and Laylah discussed the effects of irresponsibility, explaining that irresponsibility can result in long term and short term effects. QMHP-C provided Laylah with Laylah with tips on behaving more responsibility such as being accountable, setting and achieving goals, exercising control and setting a good example. 1. Response: Laylah was able to categorize her characteristics as positive or negative. She participated in the role play scenarios and discussions and demonstrated positive behavior characteristics. She needed assistance with the definition of some terms such as empathy. 2. Response: Laylah was able to identify a few irresponsible behaviors but was more responsive with explaining why a behavior was considered irresponsible and the effects of irresponsible behavior. She displayed active listening skills while reviewing the worksheet on ways to be more responsible.
